Requirements

RN CAP 2 criteria or advancement from RN CAP 1 (entry level into Clinical Advancement Program)
Ability to perform heavy lifting, moving of patients, assisting with ambulation, frequent prolonged standing, bending over
Capability to move (with assistance) machines/equipment up to 200 lbs. and lift chemical/water solutions up to 30 lbs. as high as 5 feet
Completion of CAP requirements to maintain or advance
Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations

Responsibilities

Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities
Ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, documentation in medical record, adjustments to treatment plan, and notification to Team Leader/Charge Nurse/Supervisor/Physician as needed
Assessment, collaboration, and documentation of patient/family basic learning needs; provision of initial and ongoing education
Direction and provision (in collaboration with staff) of safe and effective daily hemodialysis therapy
Administration of medications as prescribed or per approved algorithm(s), with documentation of justification and effectiveness
Initiation or assistance with emergency response measures
Serving as resource, leader, coach, mentor, and role model for new and incumbent employees
Ensuring correct laboratory collection, processing, shipping procedures; rescheduling missed/insufficient collections
Identification of expected outcomes, documentation, and updating of nursing assessment and plan of care via collaboration with Interdisciplinary Team
Ensuring patient awareness of transplant and treatment modality options
Participation in education and quality improvement projects at facility and area level
Serving as Preceptor to new employees
Performance of all other duties as assigned by Supervisor
