Requirements

  • RN CAP 2 criteria or advancement from RN CAP 1 (entry level into Clinical Advancement Program)
  • Ability to perform heavy lifting, moving of patients, assisting with ambulation, frequent prolonged standing, bending over
  • Capability to move (with assistance) machines/equipment up to 200 lbs. and lift chemical/water solutions up to 30 lbs. as high as 5 feet
  • Completion of CAP requirements to maintain or advance
  • Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations

Responsibilities

  • Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, documentation in medical record, adjustments to treatment plan, and notification to Team Leader/Charge Nurse/Supervisor/Physician as needed
  • Assessment, collaboration, and documentation of patient/family basic learning needs; provision of initial and ongoing education
  • Direction and provision (in collaboration with staff) of safe and effective daily hemodialysis therapy
  • Administration of medications as prescribed or per approved algorithm(s), with documentation of justification and effectiveness
  • Initiation or assistance with emergency response measures
  • Serving as resource, leader, coach, mentor, and role model for new and incumbent employees
  • Ensuring correct laboratory collection, processing, shipping procedures; rescheduling missed/insufficient collections
  • Identification of expected outcomes, documentation, and updating of nursing assessment and plan of care via collaboration with Interdisciplinary Team
  • Ensuring patient awareness of transplant and treatment modality options
  • Participation in education and quality improvement projects at facility and area level
  • Serving as Preceptor to new employees
  • Performance of all other duties as assigned by Supervisor
