Registered Nurse (RN) license in good standing
Completion of the Clinical Advancement Program (CAP) requirements
Ability to perform ongoing assessment, planning, intervention, and evaluation of patient care
Knowledge of hemodialysis procedures and protocols
Ability to collaborate with interdisciplinary team members
Strong communication and interpersonal skills

Responsibilities

Performs ongoing systematic collection and analysis of patient data pre, during, and post hemodialysis treatment
Documents in the patient medical record and adjusts treatment plans as needed
Notifies Team Leader, Charge Nurse, Supervisor, or Physician of any concerns
Assesses, collaborates, and documents patient/family’s basic learning needs and provides initial and ongoing education
Directs and provides hemodialysis therapy to assigned patients in collaboration with other patient care staff
Administers medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness
Initiates or assists with emergency response measures
Serves as a resource for the healthcare team
Participates in staff training and orientation of new staff
Ensures correct laboratory collection, processing, and shipping procedures are performed
Identifies expected outcomes and updates the nursing assessment and plan of care
Ensures patient awareness related to transplant and treatment modality options
Performs all other duties as assigned by the Supervisor
