  • Understanding of End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, laboratory values, Sodium (Na) modeling, and Ultrafiltration modeling

Responsibilities

  • Direct Patient Care Technicians in safe and effective delivery of chronic hemodialysis therapy per facility policies, corporate standards, state, and federal regulations
  • Delegate tasks to direct patient care staff (LVN/LPNs, Patient Care Technicians, Dialysis Assistants)
  • Assess daily patient care needs and develop appropriate patient care assignments
  • Routinely monitor patient care staff for appropriate techniques and adherence to policies/procedures
  • Participate in staff training, orientation of new staff, and required staff meetings
  • Ensure educational needs of patients and families are met regarding ESRD, renal disease, vascular access, dialysis therapy, related health conditions, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, and laboratory values
  • Provide safe and effective delivery of care to ESRD patients, including accurate implementation of treatment prescriptions (Sodium modeling, Ultrafiltration modeling)
  • Assess patients’ response to hemodialysis, make adjustments/modifications to treatment plans as indicated, and communicate issues to Team Leader or physician
  • Identify and communicate patient-related issues to Team Leader or physician
  • Initiate Initial and Annual Nursing Assessments, ongoing evaluation, and documentation of patient care needs per FMC policies
  • Actively participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments
  • Take appropriate interventions for changes in patient adequacy status and troubleshoot access flow issues (e.g., OLC/AMP yellow lights)
  • Provide, supervise (if applicable), and monitor hemodialysis access care per established procedures
  • Implement, administer, monitor, and document patient responses to prescribed interdialytic transfusions, notifying physician and blood supplier of adverse reactions
  • Ensure accurate and complete documentation by Patient Care Technicians on Hemodialysis Treatment Sheets
  • Review, transcribe, and enter physician lab orders accurately into the Medical Information System
  • Ensure appropriate preparation of lab requisitions for Spectra or other systems
