Requirements

  • Entry level designation into the Clinical Advancement Program (CAP)
  • Ability to perform all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses
  • Registered Nurse (RN) licensure (implied by job title and duties)
  • Ability to handle physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Tolerance for work environment: health care facility with air temperature control, moderate noise, exposure to infectious/contagious diseases, desk/computer work, patient/staff/physician interaction, possible travel
  • Required to complete CAP requirements to advance

Responsibilities

  • Performs ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment for assigned patients, documents in medical record, makes adjustments to treatment plan, notifies Team Leader/Charge Nurse/Supervisor/Physician as needed
  • Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education
  • Directs and provides, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
  • Administers medications as prescribed or per approved algorithm(s), documents justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource for health care team, participates in staff training and orientation of new staff
  • Ensures correct laboratory collection, processing, shipping; reschedules missed/insufficient collections
  • Identifies expected outcomes, documents and updates nursing assessment and plan of care via collaboration with Interdisciplinary Team
  • Ensures patient awareness of transplant and treatment modality options
  • Participates in decision-making, teaching, leadership functions, and quality improvement activities
  • Performs all other duties as assigned by Supervisor
  • All duties performed in accordance with Fresenius Kidney Care policy, procedures, nursing practice standards, state/federal regulations
