Requirements

RN CAP 2 criteria or advancement from RN CAP 1 (entry level into Clinical Advancement Program)
Accountability for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation
Ability to delegate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership, and quality improvement activities
Completion of CAP requirements to maintain or advance
Physical ability for heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, and lifting/moving equipment up to 200 lbs. (with assistance) and solutions up to 30 lbs

Responsibilities

Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust treatment plan, and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education
Direct and provide, in collaboration with staff, all aspects of daily safe and effective hemodialysis therapy delivery
Administer medications as prescribed or per approved algorithm(s), document justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for employees
Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
Identify expected outcomes, document, and update nursing assessment and plan of care via Interdisciplinary Team collaboration
Ensure patient awareness of transplant and treatment modality options
Participate in education and quality improvement projects
May serve as Preceptor to new employees
Perform all other duties as assigned by Supervisor
