Requirements

  • RN CAP 2 criteria or advancement from RN CAP 1 (entry level for new employees meeting criteria)
  • Ability to perform under direction of Supervisor, with guidance from Clinical Educator/Preceptor, or in collaboration with other RNs
  • Capability for heavy lifting, moving patients, assisting with ambulation (with equipment aids/coworkers)
  • Frequent, prolonged standing; ability to bend over
  • Occasional movement (with assistance) of machines/equipment up to 200 lbs.; lift chemical/water solutions up to 30 lbs. as high as 5 feet
  • Tolerance for healthcare facility environment (air temperature control, moderate noise); potential exposure (description incomplete)
  • Completion of CAP requirements to maintain or advance

Responsibilities

  • Provision and coordination of clinically competent care: assessment, planning, intervention, evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership, and quality improvement activities as part of ESKD health care team
  • Ongoing, systematic collection/analysis of patient data pre/during/post hemodialysis; document in medical record; adjust treatment plan; notify Team Leader/Charge Nurse/Supervisor/Physician as needed
  • Assess, collaborate, document patient/family learning needs; provide initial/ongoing education
  • Direct/provide (with staff) safe/effective daily hemodialysis therapy delivery
  • Administer medications per prescription/approved algorithm; document justification/effectiveness
  • Initiate/assist with emergency response measures
  • Serve as resource, leader, coach, mentor, role model for employees (behavior, work habits, attitudes)
  • Ensure correct lab collection, processing, shipping; reschedule missed/insufficient collections
  • Identify expected outcomes; document/update nursing assessment/plan of care via Interdisciplinary Team collaboration
  • Ensure patient awareness of transplant/treatment modality options
  • Participate in education/quality improvement projects at facility/area level
  • Serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
  • Comply with Fresenius Kidney Care policy/procedures, nursing standards, state/federal regulations
