Requirements

  • Entry level designation into the Clinical Advancement Program (CAP)
  • Accountable and responsible for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Completion of CAP requirements to advance
  • Physical ability to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs., lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Work in a health care facility environment with potential exposure to infectious/contagious diseases, moderate noise, air temperature control; includes desk/computer work, patient/staff/physician interaction, possible travel
  • Assigned oversight of Patient Care Technicians

Responsibilities

  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
  • Perform essential functions under direction of Supervisor and with guidance from Clinical Educator, Preceptor, or in collaboration with other RNs
  • Perform ongoing systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, make adjustments/modifications to treatment plan, notify Team Leader/Charge Nurse/Supervisor/Physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education
  • Direct and provide, in collaboration with direct and ancillary staff, all aspects of daily safe and effective hemodialysis therapy delivery
  • Administer medications as prescribed or per approved algorithm(s), document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource for health care team, participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, and shipping; reschedule missed/insufficient collections
  • Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Perform all other duties as assigned by Supervisor
