Entry level designation into the Clinical Advancement Program (CAP)
Ability to perform direct patient care involving heavy lifting, moving of patients, assisting with ambulation, frequent prolonged standing, bending over, moving machines/equipment up to 200 lbs. (with assistance), and lifting chemical/water solutions up to 30 lbs. as high as 5 feet
Comfortable in a health care facility work environment with air temperature control, moderate noise levels, potential exposure to infectious/contagious diseases/materials
Capable of desk work, computer work, interaction with patients, facility/hospital staff, and physicians; may require travel to training sites or other facilities
Required to complete CAP requirements to advance
Assigned oversight of Patient Care Technicians

Responsibilities

Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for an assigned group of patients
Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities as a member of the End Stage Kidney Disease (ESKD) health care team
Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
Perform essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses
Ongoing systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in patient medical record, make adjustments/modifications to treatment plan, notify Team Leader/Charge Nurse/Supervisor/Physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education to patients and family
Direct and provide (in collaboration with direct and ancillary patient care staff) all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients
Administer medications as prescribed or in accordance with approved algorithm(s); document appropriate medical justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for health care team; participate in staff training and orientation of new staff as assigned
Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient laboratory collections
Identify expected outcomes, document and update the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team
Ensure patient awareness related to transplant and treatment modality options
Perform all other duties as assigned by Supervisor
