Requirements

Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between May 2024 and September 2025
Currently residing in or willing to relocate to program location (Marietta, GA) for at least the next two years
Have not yet worked as an NP
Minimum of one year experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 3/16/2026 an active, non-probationary state NP license

Responsibilities

Commit to working at Oak Street Health for at least one year following program completion
Participate in the 12-month paid fellowship program, including building your own patient panel
Learn from a multitude of preceptors
Engage in didactic content that builds on graduate training
Spend time in the value-based care model providing care to older adults
