The position is onsite at 3621 Aramingo Ave, Philadelphia, PA 19134.

Candidates need a Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate, national certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner, active non-probationary state NP license, and active DEA license. Bilingual Spanish is preferred.

Nurse Practitioners provide value-based primary care to adults on Medicare, see patients independently or collaborate with physicians, build meaningful patient relationships, and work closely with the care team, operational leadership, and report to the Center Medical Director.

Strong candidates will have the required advanced nursing degree and certifications, active licenses, and preferably be bilingual in Spanish, with a focus on delivering exceptional primary care, patient education, care coordination, and population health leadership.
