Requirements

  • Functions as a Registered Nurse Applicant under the direction or supervision of a Staff Registered Nurse
  • Obtain nursing license and registration within the timeframe outlined in state specific regulations or within 90 days from hire date, whichever comes sooner
  • Adheres to FMS policies, procedures, training, FMCNA Compliance Program, patient care and administrative policies
  • Supports FMCNA’s mission, vision, values, customer service philosophy, Quality Enhancement Program (QEP), CQI Activities, and process improvement activities

Responsibilities

  • Directs Patient Care Technician’s provision of safe and effective chronic hemodialysis therapy in compliance with facility policies, procedures, and regulations
  • Delegates tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants
  • Assesses daily patient care needs and develops appropriate patient care assignments
  • Routinely monitors patient care staff for appropriate techniques and adherence to facility policy and procedures
  • Participates in staff training, orientation of new staff, and all required staff meetings
  • Ensures educational needs of patients and family are met regarding End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, diet/fluid and medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, laboratory values, and compliance
  • Provides safe and effective delivery of care to patients with ESRD, including accurate implementation of treatment prescriptions (Sodium modeling, Ultrafiltration modeling)
  • Assesses patients’ response to hemodialysis treatment, makes adjustments/modifications to treatment plan, and communicates issues to Team Leader or physician
  • Identifies and communicates patient related issues to Team Leader or physician
  • Initiates Initial and Annual Nursing Assessment, and ongoing evaluation and documentation of patient care needs
  • Actively participates in pre evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of hemodialysis treatments
  • Takes appropriate intervention for changes in patient adequacy status and troubleshooting access flow issues
  • Provides, supervises, and monitors hemodialysis access care
  • Implements, administers, monitors, and documents patient’s response to prescribed interdialytic transfusions, including notification of adverse reactions
