Requirements

  • Functions as Registered Nurse Applicant under direction/supervision of Staff Registered Nurse
  • Obtain nursing license and registration within state-specific timeframe or 90 days from hire, whichever sooner
  • Adheres to FMS/FMCNA policies, procedures, training, compliance program, and patient care/administrative policies
  • Supports FMCNA mission, vision, values, customer service philosophy, QEP, CQI activities, patient satisfaction, and process improvements

Responsibilities

  • Directs Patient Care Technicians in safe/effective chronic hemodialysis therapy delivery per facility policies, corporate/state/federal regulations
  • Delegates tasks to direct patient care staff (LVN/LPNs, Patient Care Technicians, Dialysis Assistants)
  • Assesses daily patient care needs and develops appropriate assignments
  • Monitors patient care staff for techniques and adherence to policies/procedures
  • Participates in staff training, orientation of new staff, and required staff meetings
  • Ensures educational needs of patients/family met re: ESRD, renal disease, vascular access, dialysis therapy, related conditions, diet/fluid/medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, lab values
  • Provides safe/effective dialysis treatment: implements prescriptions (Na modeling, Ultrafiltration), assesses patient response, adjusts treatment, communicates issues to Team Leader/physician
  • Identifies/communicates patient-related issues to Team Leader/physician
  • Initiates Initial/Annual Nursing Assessment, ongoing evaluation/documentation of patient care needs per FMC policies
  • Participates in pre-evaluation, initiation, monitoring, termination, access homeostasis, post-evaluation of hemodialysis therapy per procedures
  • Intervenes for patient adequacy changes/troubleshooting access flow issues (OLC/AMP)
  • Provides/supervises/monitors hemodialysis access care per procedures
  • Implements/administers/monitors/documents response to interdialytic transfusions, notifies of adverse reactions
