Requirements

  • Functions as Registered Nurse Applicant under direction/supervision of Staff Registered Nurse
  • Obtain nursing license and registration within state-specific timeframe or 90 days from hire, whichever sooner
  • Adhere to FMS policies, procedures, training, FMCNA Compliance Program, and patient care/administrative policies
  • Support FMCNA mission, vision, values, customer service philosophy, Quality Enhancement Program (QEP), CQI activities, and process improvements

Responsibilities

  • Staff Related
  • Direct Patient Care Technicians in safe/effective chronic hemodialysis therapy delivery per policies, regulations
  • Delegate tasks to direct patient care staff (LVN/LPNs, Patient Care Technicians, Dialysis Assistants)
  • Assess daily patient care needs and develop appropriate assignments
  • Monitor patient care staff for techniques and adherence to policies/procedures
  • Participate in staff training, orientation of new staff
  • Attend all required staff meetings

  • Meet educational needs of patients/family on End Stage Renal Disease (ESRD)
  • Provide ongoing education on renal disease, vascular access, dialysis therapy, related health conditions
  • Discuss/record education on diet/fluid/medication compliance
  • Provide detailed education on adequacy measures (OLC, AMP, UKM)
  • Ensure transplant/modality awareness, drive catheter reduction
  • Educate on laboratory values relation to dialysis therapy, compliance, medications, fluid
  • Dialysis Treatment
  • Provide safe/effective care to ESRD patients
  • Implement treatment prescriptions (Sodium modeling, Ultrafiltration modeling)
  • Assess patient response to hemodialysis, adjust/modify plan, communicate issues to Team Leader/physician
  • Identify/communicate patient issues to Team Leader/physician
  • Initiate/conduct Initial/Annual Nursing Assessment, ongoing evaluation/documentation per policies
  • Participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, post-evaluation of hemodialysis
  • Intervene for patient adequacy changes, troubleshoot access flow issues (OLC/AMP)
  • Provide/supervise/monitor hemodialysis access care per procedures
  • Implement/administer/monitor/document interdialytic transfusions, notify of adverse reactions
