Requirements

  • Functions as part of the hemodialysis health care team as a Registered Nurse Applicant under the direction or supervision of a Staff Registered Nurse
  • Obtain nursing license and registration within the timeframe outlined in state specific regulations or within 90 days from hire date, whichever comes sooner
  • Adhere to FMS policies, procedures, training, FMCNA Compliance Program, patient care and administrative policies
  • Support FMCNA’s mission, vision, values, customer service philosophy, Quality Enhancement Program (QEP), CQI Activities, patient satisfaction, and process improvement activities

Responsibilities

  • Direct Patient Care Technician’s provision of safe and effective chronic hemodialysis therapy in compliance with facility policies, procedures, and regulations
  • Delegate tasks to direct patient care staff including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants
  • Assess daily patient care needs and develop appropriate patient care assignments
  • Routinely monitor patient care staff for appropriate techniques and adherence to facility policy and procedures
  • Participate in staff training, orientation of new staff, and all required staff meetings
  • Ensure educational needs of patients and family are met regarding End Stage Renal Disease (ESRD), renal disease, vascular access, dialysis therapy, related health conditions, diet/fluid and medication compliance, adequacy measures (OLC, AMP, UKM), transplant/modality awareness, catheter reduction, laboratory values, and dialysis compliance
  • Provide safe and effective delivery of care to patients with ESRD, including accurate implementation of treatment prescriptions (Sodium modeling, Ultrafiltration modeling)
  • Assess patients’ response to hemodialysis treatment, make adjustments/modifications to treatment plan as indicated, and communicate problems/concerns to Team Leader or physician
  • Identify and communicate patient-related issues to Team Leader or physician
  • Initiate Initial and Annual Nursing Assessment, and ongoing evaluation/documentation of patient care needs per FMC policies
  • Actively participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatment per established procedures
  • Take appropriate intervention for changes in patient adequacy status and troubleshoot access flow issues (e.g., OLC/AMP yellow lights)
  • Provide, supervise (if applicable), and monitor hemodialysis access care per established procedures
  • Implement, administer, monitor, and document patient’s response to prescribed interdialytic transfusions, including notification of adverse reactions
