Requirements

  • Current NJ Paramedic certification issued by the NJ Department of Health
  • Minimum 1 year field experience as an active paramedic
  • High school diploma or GED
  • Graduate of a recognized Paramedic Training Program
  • Current BLS CPR certification (American Heart Association or equivalent)
  • Current American Heart Association Advanced Cardiac Life Support (ACLS) certification
  • Current American Heart Association Pediatric Advanced Cardiac Life Support (PALS) or American Academy of Pediatrics PEPP certification
  • Emergency Vehicle Operator Course (EVOC) certification
  • Valid driver’s license recognized by the State of New Jersey; driving record must meet insurance company standards
  • Prior MIH, hospital-at-home, home health, chronic care management, or telehealth/remote monitoring experience preferred

Responsibilities

  • Conducts comprehensive in-home patient assessments, medication reconciliation, and administers treatments within the paramedic scope of practice; identifies clinical changes and documents plan of care
  • Completes accurate clinical documentation in the EMR/ePCR; responds to quality assurance and medical oversight requests
  • Facilitates telehealth visits with providers, sets up and manages remote monitoring devices, and communicates findings to the interdisciplinary care team
  • Maintains MIH vehicles, durable medical equipment, and supply inventory; ensures infection control and equipment cleaning after each visit
  • Performs point-of-care testing and specimen collection (phlebotomy) per protocol and coordinates specimen transport/processing
  • Participates in patient/caregiver education, community outreach, and program improvement activities; cross-trains with hospital-at-home staff
  • Performs scheduled in-home clinical care and post-discharge follow-up as part of the Mobile Integrated Healthcare (MIH) program, including home safety inspection, patient assessment, medication reconciliation, point-of-care testing, telehealth facilitation, and equipment setup to support safe transition of care and reduce readmissions
