Candidates must hold a Master’s degree in social work from a Council on Social Work Education accredited program, be licensed as a licensed clinical social worker by the Board of Behavioral Sciences, possess at least one year of experience in healthcare, preferably with home health care experience, demonstrate excellent verbal and written communication skills, exhibit strong organizational abilities, and maintain current CPR certification. A valid driver’s license and access to a reliable, insured vehicle are also required.
As a Medical Social Worker, the individual will conduct psychosocial assessments of patients, develop and implement tailored intervention plans, maintain accurate clinical records, offer information and referral services to patients, families, and caregivers, and serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.
