Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Licensed as a Social Worker within the scope of clinical training and state license (implied for providing counseling services)
  • Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
  • Familiarity with FKC patient education programs, social work focused interventions, and quality of life measurement instruments
  • Current knowledge of local vocational/educational rehabilitation programs, community resources (e.g., home health services), and advance directives
  • Adherence to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment within the interdisciplinary team
  • Participate in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and improved quality of life
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Offer supportive counseling services to patients within scope of clinical training and state license
  • Provide educational and goal-directed counseling to patients seeking transplant
  • Assist with referrals to community resources (e.g., home health, vocational rehabilitation) and facilitate access to vocational/educational programs to support employment or education
  • Assess patient awareness of advance directives, assist with accessing forms/information, and facilitate discussions with the healthcare team, patient, and family
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
  • Provide psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns based on acuity level
  • Participate in monthly Quality review meetings, reporting on indicators like Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
  • Assess patient knowledge of kidney disease for barriers to adherence and provide tailored education with patient, family, and healthcare team
  • Document MSW interactions and interventions provided to patient and/or family
  • Collaborate with interdisciplinary team to inform, educate, and support staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease
  • Actively participate in QAI activities and process improvements to achieve FKC Quality Goals, including patient satisfaction and quality of life
