Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Adheres to FMCNA Compliance Program, regulatory requirements, FMS policies, and procedures
  • Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
  • Knowledgeable of FMCNA Social Work Policy, including documentation
  • Utilizes FMS Ultra Care patient education programs, quality of life measurement instruments, and established outcomes-driven psycho-educational counseling methods
  • Supports FMCNA mission, core values, expected behaviors, customer service philosophy, Quality Indicators, Outcomes, QAI activities, and process improvement

Responsibilities

  • Drives FMS culture through values and customer service standards; accountable for outstanding customer service to external and internal customers
  • Develops and maintains effective relationships through effective and timely communication
  • Takes initiative to respond, resolve, and follow up on customer service issues timely
  • Assesses patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment in interdisciplinary team
  • Participates in care planning with patient and healthcare team to identify interventions for rehabilitation goals and improved quality of life
  • Provides supportive and goal-directed counseling, including for transplant-seeking patients, non-adherence, and quality of life concerns (usual care or Social Work Intensive)
  • Assesses patient awareness of advance directives; assists with accessing forms/information and facilitates discussions with team, patient, and family/support persons
  • Provides general information about Do Not Resuscitate Orders and Advanced Directives
  • Participates in discussions of patient DNR status with physician and nurse to ensure understanding and informed decision-making
  • Provides information and assists with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal treatment outcomes
  • Assesses patient knowledge of kidney disease and barriers to adherence; provides tailored education with patient, family, and healthcare team
  • Informs, educates, and supports staff on emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
  • Actively participates in process improvement activities to enhance achievement of FMCNA Quality Goals, including patient satisfaction and quality of life
