Requirements

Master's degree in Social Work (MSW)
Current state licensure as a Licensed Master Social Worker (or equivalent, as permitted by state regulations)
Knowledge of social work theory of human behavior and accepted methods of social work practice
Familiarity with Chronic Kidney Disease's emotional, psychological, and behavioral impacts on patients and families
Knowledge of FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
Current knowledge of local vocational/educational rehabilitation programs and community resources (e.g., home health services)
Ability to utilize FKC patient education programs, social work focused interventions, and quality of life measurement instruments
Clinical training in supportive counseling within scope of license

Responsibilities

Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment
Participate in care planning with the interdisciplinary team, patient, and family to identify interventions for rehabilitation, treatment goals, and quality of life
Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
Offer supportive counseling services to patients within scope of clinical training and state license
Provide educational and goal-directed counseling to patients seeking transplant
Assist with referrals to community resources (e.g., home health, vocational rehabilitation) to facilitate optimal treatment outcomes
Assess patient awareness of advance directives, facilitate access and discussions with team and family
Participate in discussions of patient DNR status with physician, nurse, patient, and family
Document MSW interactions and interventions provided to patient and/or family
Provide psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns
Participate in monthly Quality review meetings, reporting on indicators like Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
Assess patient knowledge of kidney disease and barriers to adherence; provide tailored education with patient, family, and team
Educate and support staff on the emotional, psychological, and behavioral impacts of Chronic Kidney Disease
Actively participate in QAI activities and process improvements to achieve FMCNA Quality Goals, including patient satisfaction and quality of life
