Requirements

RN license and minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role (or equivalent combination of education and experience)
Current, unrestricted RN license in applicable state(s)
May require state-specified certification based on state law and/or contract
Reside in Richmond, Henrico, or Chesterfield Counties, VA (field position requiring travel to client sites and occasional office attendance)

Responsibilities

Overall management of member's case within scope of licensure, including supervision and direction to non-RN clinicians per state law and contract
Develop, monitor, evaluate, and revise member's care plan to meet needs and optimize health care across the care continuum
Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination, and management of member's needs (physical health, behavioral health, social services, long-term services and supports)
Identify members for high-risk complications and coordinate care with member and health care team
Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits
Obtain thorough and accurate member history to develop individual care plan; establish short- and long-term goals with member, caregivers, family, natural supports, physicians
Identify members benefiting from alternative level of care or waiver programs; ensure member's access to services
Assist with implementation of care plans by facilitating authorizations/referrals within benefits or extra-contractual arrangements
Interface with Medical Directors, Physician Advisors, and/or Interdisciplinary Teams on care management treatment plans; assist in problem-solving with providers, claims, or service issues
Direct/supervise LPN/LVN, LSW, LCSW, LMSW, and other non-RN licensed professionals by assigning tasks, verifying/interpreting information, conducting assessments, developing/monitoring/revising care plans, and reviewing performance
