This is a field-based role in the Dallas Ft. Worth area and Houston, TX, where associates primarily operate in the field traveling to client sites or designated locations, with occasional office attendance for meetings or training.
The role requires RN licensure for overall case management, performing telephonic or face-to-face clinical assessments, developing and revising care plans, supervising non-RN clinicians, and coordinating care for members with chronic illnesses or disabilities.
The RN provides supervision and direction to non-RN clinicians such as LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals, while interfacing with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams.
A strong candidate is a licensed RN with experience in case management, clinical assessments, care plan development, supervising non-RN staff, and coordinating multidisciplinary care for high-risk members with chronic conditions.
