Candidates should possess a high school diploma or equivalent, and demonstrate a minimum of one year of experience in a care coordination or social work setting, with a strong preference for experience working with individuals receiving long-term services and supports (LTSS). Knowledge of Iowa’s LTSS programs, including waivers such as LTSS/IDD, and familiarity with federal and state regulations are essential. Strong communication, assessment, and motivational interviewing skills are required, along with the ability to effectively collaborate with multidisciplinary teams.
The LTSS Service Coordinator will conduct face-to-face program assessments to identify individuals needing support, facilitate Person-Centered Planning processes, develop and manage comprehensive Person Centered Support Plans (PCSP) and backup plans, coordinate medical and behavioral health needs, and engage the member’s circle of support. They will also serve as a single point of contact for members, document service and support goals, identify opportunities for alternative levels of service, and provide mentorship and training to new staff, while ensuring compliance with applicable laws and regulations.
