Pharmacist - Board of Pharmacy active in state of practice (Upon Hire)
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand and distinguish speech and other sounds
Ability to work the hours specified (Day Shift)

Responsibilities

Lead the operations of the designated pharmacy department, including planning, organizing, and leading all operational functions and activities (quality, performance improvement, operational policy and procedure, financial management)
Coordinate workflow with other team members to ensure a productive and efficient environment
Oversee drug collection, reviews, audits, reports, follow up, and presentations to the P&T and Medical staff committees
Perform consults and education to healthcare professionals and patients
Coordinate all aspects of drug purchasing, contracts, agreements, and inventory management
Assist with analysis, collection, and maintenance of records as needed for all regulatory agencies
Responsible for all aspects of the Pharmacy IS system (design, maintenance, module parameters, dictionaries, formulary, operating policies and procedures, menus, personnel training, software testing, quality assurance)
Accurately interpret and evaluate physician's medication orders, compound and dispense medications, manage drug purchasing and inventory management
Evaluate medication therapy of patients and make recommendations to physicians to provide the best possible therapy while maintaining ASHP standards
Counsel patients on the proper usage, effects, and side effects of medication
Prepare program content and present educational presentations to health professionals
Identify, evaluate, and document all reported adverse drug reactions
Present completed drug reviews and audits to P&T committee and Medical staff committee, and complete follow up to action items
Provide formal pharmacy consults, pharmacokinetic consults, and nutrition consults upon request by physicians
Conduct detailed drug literature review of new drugs and make recommendations to the P&T Committee on hospital formulary status
Accountable for ongoing operations of the department regarding safety/quality standards, goals and objectives, efficiency and productivity standards, financial performance (budget and expense management), contracting and purchasing
Participate in the review, development, communication, and monitoring of department policies and procedures
