Responsibilities

Provide individual and group therapy to a panel of complex SMI patients
Supervise and mentor clinical teams, including pre-licensed therapists, licensed therapists, and dieticians, with clinical care delivery, documentation, and patient care
Manage a panel of complex members through individual and group therapy sessions
Provide psychoeducation and support to patients' families
Partner with clinical staff, peer specialists, primary care providers, and community partners in a collaborative, shared decision-making model
Act as a patient advocate, helping members navigate health and social services systems in the clinic location
Connect patients to resources and the community
Initiate, establish, and maintain relationships with patients and families built on trust and rapport
Support associate therapist team members in a supervisory capacity
