Requirements

  • Active medical licenses (MD/DO) in coverage area(s); multi-state licenses preferred
  • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
  • Execute a strong visit completion rate, pacing 35+ completed visits per month
  • Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (e.g., New London and Windham counties)
  • Flexible to travel locally and within licensed states, as needed (up to 25%)
  • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including iPad, medical supplies, and scale)
  • Accommodate field observations and/or “ridealong” experiences, as requested
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements; regular training required)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format (preferred)
  • Willingness to execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants and fulfilling all state-by-state requirements (preferred)
  • Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics

Responsibilities

  • Conduct pre-scheduled in-home visits (approximately 45 minutes each) including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
  • Use pre-populated, patient-specific forms for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens required)
  • Provide availability Monday - Sunday, with majority of visits scheduled between 8 a.m. and 7 p.m
  • Maintain training and compliance for Video Visits and DPS products
