Requirements

  • Active, unrestricted license(s) as Nurse Practitioner or Physician Assistant in coverage area(s)
  • Board certification (required)
  • Multi-state licenses (preferred)
  • Able to cover primary locations: Queens and surrounding area
  • Able to comfortably and safely perform duties in diverse home environments (e.g., with household pets, secondhand smoke, airborne allergens or irritants)
  • Flexible to travel locally and within licensed states, as needed

Responsibilities

  • Conduct in-home health evaluations (approx. 45 minutes each), including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry, bone density screening)
  • Use iPad and intuitive clinical workflow for evaluations
  • Bring empathy and build trusted relationships during visits to improve health outcomes
  • Conduct virtual visits as needed
  • Provide all Signify Health services, including Diagnostic & Preventive Services products
  • Engage one-on-one, answer health questions, and connect people to appropriate care without prescribing medicine, ordering lab tests, or altering treatment regimens
