Candidates must be a Nurse Practitioner or Physician Assistant with a current and unrestricted license to practice in Washington State. Experience in home health or a related field is preferred, and a strong understanding of medical terminology and clinical assessment skills are required.
The Per Diem Clinician will conduct in-home health evaluations, engaging with patients to assess their health needs and bridge gaps in care. They will perform physical evaluations, review medication and medical histories, and administer diagnostic tests as ordered by the patient’s health plan, such as spirometry and diabetic eye exams. Additionally, the clinician will provide education and support to patients, documenting all findings and collaborating with the Signify Health team to ensure comprehensive care.
