The Per Diem Clinician will conduct in-home health evaluations, engaging with patients and their families to assess their health needs and provide comprehensive care. They will review medication and medical histories, perform physical evaluations, and order diagnostic tests as needed, while adhering to established clinical workflows and utilizing an iPad for documentation. Additionally, they will participate in virtual visits and provide education to patients regarding their health conditions and treatment plans.
