Requirements

HBO-V diploma, aangevuld met een erkende, afgeronde vervolgopleiding op het gebied van neurologie of revalidatie (of bereid deze op korte termijn te volgen)
Ruime werkervaring c.q. affiniteit met revalidatie en/of neurologische aandoeningen in zowel ziekenhuis als thuiszorgorganisaties

Responsibilities

Ondersteunen van de revalidatiearts in de coördinatie van de zorg door verschillende disciplines
Contactpersoon zijn voor de cliënt, naasten en andere zorgverleners, in het bijzonder voor de thuiszorg
Geven van voorlichting en advies en begeleiden van cliënten bij waarde gerichte keuzes, zingeving en omgaan met toenemende beperkingen
Streven naar kwaliteit van leven voor de cliënt en diens naasten
Betrokken zijn bij zorg rondom (in)continentie, wonden en PEG/PRG sonde
Betrokken zijn bij de cliënt vanaf diagnosestelling tot aan het levenseinde, inclusief monitoren van het ziektebeloop en problematiek met behulp van de zorgapp ALS@HOME
Deel uitmaken van ALS-teams in de regio Gouda, Den Haag en Leiden
Ondersteunen bij het organiseren van cliënt- en/of partnerbijeenkomsten
Deelnemen aan MDO van het ALS-(NMA-)team
Meewerken aan onderzoeken, scholing, deskundigheidsbevordering en deelname aan regionale of landelijke netwerken
Begeleiding van cliënten in de 2de lijn als onderdeel van het multidisciplinair team, met overleg met revalidatiearts en andere disciplines
Begeleiding van cliënten in de 1e lijn, met overleg met huisarts en andere betrokken disciplines
Werken bij Basalt op de polikliniek en bij zorgvragers thuis
