Requirements

Licensed Practical Nurse (LPN) - Active license in the state of practice from the Board of Nursing upon hire
Minimum of 3-5 years of experience
Visual acuity adequate to perform position duties
Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
Ability to work flexible rotating shifts: days/evenings/nights/weekends (e.g., 6:00am-6:30pm or 6:00pm-6:30am) on a PRN basis

Responsibilities

Perform professional nursing care to residents according to their needs and supervise nursing activities of the facility
Conduct daily rounds and communicate with appropriate team members during the shift; contact providers and maintain ongoing information exchange
Administer routine medications, document administration, make decisions on PRN medications, and obtain specimens for lab tests as ordered
Complete documentation thoroughly and timely, including all medications and treatments administered; participate in developing the interdisciplinary plan of care
Participate in interdisciplinary team meetings; initiate, participate, and coordinate additional meetings for crises or team support
Maintain departmental policies, procedures, quality improvement, environmental safety, and infection control standards
Initiate resident referrals, transfers, and discharges
Ensure assigned CNAs/CMAs understand each resident’s plan of care; assist in training and education of nursing and support staff; participate in QAPI activities
Maintain supplies and equipment in adequate numbers and working order, following infection control policies
Provide resident and family education through direct instruction, interdisciplinary team members, or community resources
