Requirements

  • Licensed Practical Nurse (LPN) - Active license in the state of practice from the Board of Nursing upon hire
  • Minimum of 3-5 years of experience
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Ability to work flexible rotating shifts: days/evenings/nights/weekends (e.g., 6:00am-6:30pm or 6:00pm-6:30am) on a PRN basis

Responsibilities

  • Perform professional nursing care to residents according to their needs and supervise nursing activities of the facility
  • Conduct daily rounds and communicate with appropriate team members during the shift; contact providers and maintain ongoing information exchange
  • Administer routine medications, document administration, make decisions on PRN medications, and obtain specimens for lab tests as ordered
  • Complete documentation thoroughly and timely, including all medications and treatments administered; participate in developing the interdisciplinary plan of care
  • Participate in interdisciplinary team meetings; initiate, participate, and coordinate additional meetings for crises or team support
  • Maintain departmental policies, procedures, quality improvement, environmental safety, and infection control standards
  • Initiate resident referrals, transfers, and discharges
  • Ensure assigned CNAs/CMAs understand each resident’s plan of care; assist in training and education of nursing and support staff; participate in QAPI activities
  • Maintain supplies and equipment in adequate numbers and working order, following infection control policies
  • Provide resident and family education through direct instruction, interdisciplinary team members, or community resources
