Requirements

  • Registered Nurse (RN) - active license in the state of practice upon hire
  • 4-6 years in long term/skilled care or similar experience
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Ability to work the hours specified (Day Shift)

Responsibilities

  • Provide total nursing care of residents and ensure accuracy of nursing care records
  • Participate in the selection of prospective residents based on nursing care needs and nursing personnel competencies
  • Oversee all Federal/State SNF Conditions of Participation, including admissions, MDS/Care planning process, restorative nursing, and care delivery
  • Establish and implement policies/procedures for the total care of residents, inclusive of nursing activities, social services, and delegated medical care
  • Determine, along with the Administrator, the numbers and levels of nursing personnel and establish staffing patterns to provide quality nursing care 24 hours/day
  • Participate in planning, decision making, and budgeting for the facility
  • Remain aware of safety needs for staff and residents; develop policies relevant to quality of care
  • Coordinate the QAPI program and facilitate staff participation in QAPI
  • Delegate and monitor non-nursing responsibilities to other staff consistent with their training, experience, competence, legal authorization, and nursing home policy
  • Understand and coordinate Medicare/Medicaid coverage requirements
  • Assure that a comprehensive plan of care is established and implemented per regulations for each resident
  • Interact with physicians/providers to plan care for residents and assure discharge and transfer planning
  • Interview, hire, develop, train, and retain employees; plan, assign, and lead work; appraise performance; reward and coach employees; address complaints and resolve problems
  • Coordinate orientation programs for new staff and development/engagement of existing staff
  • Lead quality improvement activities; develop monitoring tools, collect data, and use data to drive improvements in care and services
