Requirements

  • Functions as a member of the interdisciplinary healthcare team
  • Supports FMCNA commitment to Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
  • Actively participates in process improvement activities
  • Adheres to established FMCNA guidelines, policies, algorithms, and Federal and state regulations, including licensure guidelines
  • Completes nutritional assessments and progress notes according to FMCNA guidelines
  • Maintains complete and accurate documentation in medical records
  • Reviews nutrition-related lab results (e.g., albumin level, body weight monthly)
  • Collaborates with interdisciplinary team on Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status, and sodium/volume control
  • Participates in interdisciplinary QAI program and reports on nutrition QAI results
  • Participates in pertinent staff meetings

Responsibilities

  • Assesses patient’s knowledge of diet and kidney disease and provides education appropriate to patient’s learning needs on the relationship of diet, kidney disease, and treatment process
  • Provides patient-specific detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Participates in interdisciplinary comprehensive assessments and development of written, individualized comprehensive plan of care
  • Calculates diet prescription according to standard of practice
  • Provides ongoing nutrition assessment and outcome-oriented nutrition counseling to assist patient in achieving and sustaining effective nutritional status
  • Identifies malnourished patients or those at risk and collaborates with interdisciplinary team on interventions, resources, or solutions
  • Implements plan of care with involvement of patient/family/caregiver
  • Monitors adherence and response to nutrition therapy, adjusts therapy as needed, documents barriers to goals, and implements plan to address issues
  • Collaborates and coordinates with team members on Pre-ESRD education
  • Works with Case Manager to coordinate plan of care for patient education and identifies candidates for nutritional supplements referral to physician
  • Reviews nutrition-related lab results, counsels patient/caregiver, formulates appropriate action, recommends treatment changes to interdisciplinary team
  • Communicates lab results and recommended treatment changes to physician, facility staff, and providers in nursing facilities (hospitals, short-term and long-term care)
