Requirements

Adheres to FMCNA Compliance Program, regulatory and FMS policy requirements
Functions as a member of the interdisciplinary healthcare team
Supports FMCNA's Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
Actively participates in process improvement activities
Knowledge of diet prescription calculation according to standard of practice
Ability to complete nutritional assessments and progress notes according to FMCNA guidelines
Skills in ongoing nutrition assessment, counseling, and monitoring therapy response
Ability to identify malnourished patients and at-risk patients
Familiarity with Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Adheres to FMCNA algorithms and medical record policies

Responsibilities

Assesses patient's knowledge of diet and kidney disease and provides appropriate education on diet's relationship to kidney disease and treatment
Provides patient-specific detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
Completes nutritional assessment and participates in interdisciplinary comprehensive assessments
Participates in developing written, individualized comprehensive plan of care per Federal and state guidelines
Calculates diet prescription according to standard of practice
Completes progress notes on all chronic dialysis patients per FMCNA guidelines
Maintains complete and accurate documentation in medical record per FMCNA policies and regulations
Provides ongoing nutrition assessment and outcome-oriented counseling to help patients achieve effective nutritional status
Identifies malnourished or at-risk patients and collaborates with interdisciplinary team on interventions
Reviews each patient's albumin level and body weight at least monthly
Implements plan of care with involvement of patient/family/caregiver
Monitors adherence and response to nutrition therapy, adjusts as needed, documents barriers, and addresses issues
Collaborates with interdisciplinary team on Urea Kinetic Modeling recommendations
Collaborates on management of mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Reports on nutrition QAI results and participates in interdisciplinary QAI program
Collaborates and coordinates with team on Pre-ESRD education
Works with Case Manager to coordinate plan of care, identifies candidates for nutritional supplements, and refers to physician as per agreements
Reviews nutrition-related lab results, counsels patient/caregiver, formulates actions, recommends treatment changes, and communicates with physician/facility staff
