Requirements

  • Adheres to FMCNA Compliance Program, regulatory and FMS policy requirements
  • Functions as a member of the interdisciplinary healthcare team
  • Supports FMCNA's Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
  • Actively participates in process improvement activities
  • Knowledge of diet prescription calculation according to standard of practice
  • Ability to complete nutritional assessments and progress notes according to FMCNA guidelines
  • Skills in ongoing nutrition assessment, counseling, and monitoring therapy response
  • Ability to identify malnourished patients and at-risk patients
  • Familiarity with Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adheres to FMCNA algorithms and medical record policies

Responsibilities

  • Assesses patient's knowledge of diet and kidney disease and provides appropriate education on diet's relationship to kidney disease and treatment
  • Provides patient-specific detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Completes nutritional assessment and participates in interdisciplinary comprehensive assessments
  • Participates in developing written, individualized comprehensive plan of care per Federal and state guidelines
  • Calculates diet prescription according to standard of practice
  • Completes progress notes on all chronic dialysis patients per FMCNA guidelines
  • Maintains complete and accurate documentation in medical record per FMCNA policies and regulations
  • Provides ongoing nutrition assessment and outcome-oriented counseling to help patients achieve effective nutritional status
  • Identifies malnourished or at-risk patients and collaborates with interdisciplinary team on interventions
  • Reviews each patient's albumin level and body weight at least monthly
  • Implements plan of care with involvement of patient/family/caregiver
  • Monitors adherence and response to nutrition therapy, adjusts as needed, documents barriers, and addresses issues
  • Collaborates with interdisciplinary team on Urea Kinetic Modeling recommendations
  • Collaborates on management of mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Reports on nutrition QAI results and participates in interdisciplinary QAI program
  • Collaborates and coordinates with team on Pre-ESRD education
  • Works with Case Manager to coordinate plan of care, identifies candidates for nutritional supplements, and refers to physician as per agreements
  • Reviews nutrition-related lab results, counsels patient/caregiver, formulates actions, recommends treatment changes, and communicates with physician/facility staff
