Registered Nurse (RN) licensure
Competency in operation of home dialysis therapy-related equipment, including troubleshooting via telephone
Appropriate staff licensures and CPR certifications
Ability to manage physical demands of a health care facility work environment

Responsibilities

Manages operations of multiple Home Therapies programs, including growth, patient retention, program costs, operational methods, scheduling, and staffing
Directs activities related to strategic plans, goals, and objectives, aligning with region, division, and company
Implements quality goals and action plans to achieve quality standards
Collaborates with operations leaders, market development, in-center leadership, and patient education/quality teams to promote home therapies growth
Manages profit and loss, ensuring optimal performance to achieve or exceed budgets and key performance indicators
Directs and manages all aspects of patient care for Home Therapies patients from admission through discharge
Provides direction, guidance, and support to staff for safe and effective delivery of home therapy care to ESRD patients
Oversees patient and caregiver readiness, ability to perform dialysis at home, assesses home environment suitability, and trains on equipment operation and maintenance
Manages patient experience efforts, supporting satisfaction initiatives and embodying company values
Ensures staff competency in equipment operation, troubleshooting, licensures, and certifications
Manages program supplies and equipment tracking, ensuring timely replenishment
Directs Continuous Quality Assurance Improvement, patient retention, Clinical Algorithm Adoption, patient access management, and patient training success
Directs data entry activities (e.g., patient admission, regulatory reporting, billing, physician orders, lab results, vital signs, treatment data)
Ensures regulatory, compliance, and audit activities are completed on time
Orients and supports new home therapies staff, provides ongoing education, and ensures compliance with risk management initiatives (including in-center staff)
Handles hiring, coaching, counseling, performance reviews, disciplinary action, and terminations
Provides leadership, coaching, and development plans for direct reports, partnering with HR
Performs other related duties as assigned
