Requirements

  • Functions under direct supervision of a licensed nurse in accordance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process, and vascular access
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in operating hemodialysis machines, including setup, programming for Ultrafiltration (UF) goal, treatment time, UF modeling, and enabling safety alarms (e.g., Narrow Venous Limits at 160)
  • Ability to obtain vital signs, weights, and perform pre/post dialysis evaluations
  • Proficient in documentation on Hemodialysis Treatment Sheet and progress notes
  • Commitment to Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvement
  • Ability to maintain a safe and clean working environment and conduct machine safety checks

Responsibilities

  • Assist in providing patient-specific education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate, and administer heparin as allowed
  • Obtain pre/post treatment vital signs, weights, and vascular access evaluations
  • Initiate, monitor, and discontinue dialysis treatments per prescribed orders, including blood flow (QB), dialysate flow (QD), and reinfusion procedures
  • Evaluate intradialytic problems, intervene as prescribed, monitor patient response, and report changes to RN/supervisor
  • Calculate and enter patient-specific dialysis machine programming (UF goal, treatment time, UF modeling)
  • Record vital signs, treatment parameters, and evaluations accurately and timely with initial identification
  • Document interventions, patient responses, and any unusual findings; complete Hemodialysis Treatment Sheet and progress notes
  • Perform hemostasis, apply dressings, evaluate patient prior to discharge (standing/sitting BP)
  • Obtain prescribed lab testing, prepare specimens, clean blood spills per policy
  • Prepare/organize supplies and equipment efficiently to prevent wastage
  • Monitor patients performing self-care under RN supervision and assist as applicable
  • Observe patients, conduct machine safety checks, and report issues to RN/supervisor
  • Evaluate vascular access during treatment (arterial/venous pressures), intervene as needed, and document/report findings
