Requirements

  • Associate’s degree and two years’ experience in providing human services, or five years of human service experience, or possess a relevant professional credential (e.g., Certified Psychiatric Rehabilitation Practitioner, Bachelor’s degree in Social Work, Certified Rehabilitation Counselor, Certified Alcohol and Drug Counselor, Bachelor’s degree in Rehabilitation, or Bachelor of Science in Nursing)
  • Must have and maintain a valid driver’s license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances
  • Must be able to analyze work, set goals, develop plans, and utilize time effectively and efficiently

Responsibilities

  • Plans and provides group services around consumers’ IRP treatment needs
  • Services include: engagement strategies, activities to meet meaningful goals, Illness Management and Recovery Program, skill development, prevocational services, counseling, learning activities, social/leisure services, and offsite interventions
  • Maintains authorization timeframes, intake and daily documentation, and service planning
  • Demonstrates competent written and verbal communication with referral sources and external customers
  • Provides attentive and responsive support to customer needs
  • Works collaboratively with coworkers and agency staff
  • Meets program obligations (contractual, licensing, Medicaid, etc.)
  • Follows program policies and procedures
  • Educates self on issues impacting the program population
  • Operates conscientiously within the program budget (client engaging funds, office supplies)
