Requirements

  • High School Diploma

Responsibilities

  • Develop relationships, conduct preventative health screenings, provide health-related education and resource information to members of the community
  • Educate and/or enroll patients and/or community members to needed programs and services, track appointments, and provide follow-up for missed appointments and opportunities, complete data as needed
  • General clerical duties, inventory/order of supplies, attend resource events and display set up
  • Support the Mobile Farmer’s Market by delivering fresh food directly to neighborhoods and working closely with the mobile unit collaborative team
