  • Ability to serve as a bridge between ECM Students and the healthcare system
  • Field-based capability for contacts including accompaniment to appointments
  • Knowledge of evidence-based approaches such as Motivational Interviewing
  • Ability to engage Students in-person, by phone, in hospitals, and community settings
  • Willingness to work with prison and re-entry programs, focusing on high-need populations with complex conditions

Responsibilities

  • Perform care coordination to improve health outcomes, enhance satisfaction, and reduce unnecessary healthcare utilization
  • Coordinate medical, behavioral, and social services for ECM Students, focusing on housing, mental health, substance abuse treatment, and community resources to minimize recidivism
  • Focus support on difficult-to-engage ECM Students or those with cultural/linguistic barriers via field-based contacts and accompaniment to appointments
  • Assist RN Care Manager, BH Care Manager, and Care Coordinator in panel management by performing delegated activities
  • Assist ECM Care Team in engagement efforts for eligible ECM Students in clinics and community
  • Help Students navigate the healthcare system and community resources
  • Follow up by phone and in-person with eligible and enrolled ECM Students
  • Help Students overcome barriers to medical/behavioral health care and share barrier information with the multi-disciplinary team and providers
  • Engage Students using evidence-based approaches like Motivational Interviewing to promote collaboration with their care team
  • Collaborate on Student care issues with ECM Care Team, participate in weekly and ad hoc case reviews, and consult with RN or BH Care Manager before clinical actions
  • Accompany ECM Students to office visits as needed, per IEHP guidelines
  • Engage eligible and enrolled Students in hospitals for re-engagement post-discharge
  • Distribute health promotion materials
  • Assist with scheduling Students with other ECM Care Team members
