Requirements

BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background
Ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers
Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment
Strong verbal and written communication skills, both with virtual and in-person interactions
Attentive to details, critical thinker, and a problem-solver
Demonstrates empathy and persistence to resolve caller issues completely
Comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts
Structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary
Candidates must be within a reasonable commuting distance from posting locations (Virginia, Florida, Texas, Kansas, Iowa), unless an accommodation is granted as required by law

Responsibilities

Engage with members who have been identified as possessing significant Social Determinants of Health (SDOH) barriers
Work with the member to establish goals and next steps for addressing SDOH
Support member in selecting provider or programs most appropriate for supporting their needs
Act as a liaison between referral sources and the member
Act urgently and appropriately for members expressing suicidal ideation or homicidal ideation and members having acute psychosis
Provide primary support for the unhoused, those with serious mental illness, and substance use disorders
Performs other duties as assigned
Travels to worksite and other locations as necessary
