Responsibilities

Communicates effectively with residents, patients, families, and coworkers to protect and promote each resident’s dignity, individuality, and rights
Recognizes and honors the resident/patient’s choice, privacy, dignity, and independence; provides input in the care planning process as a member of the multidisciplinary team
Provides or assists the resident/patient with daily needs and treatments per established policy and procedure, including bathing, dressing, shaving, oral hygiene, bed making, transferring, repositioning, toileting, serving/collecting food trays, feeding, answering call lights, maintaining a clean environment, transporting patients, assisting with ambulation, and orienting/reorienting patients
Performs or assists with specific medical care activities per policy and procedure, including restorative exercises, collecting specimens, bowel and bladder training, documenting cares, intake/output, baths, dietary intake/calorie counts on flow sheets, and providing feedback to nursing staff on status changes, pain management, and concerns
Performs safety and infection control procedures, including fire, tornado, evacuation, disaster plans, universal precautions, infection control, and hazardous/chemical communication programs
Demonstrates knowledge of the facility’s strategic and operational plans and actively supports them daily, including all facility functions
Performs other duties and responsibilities as assigned
