Requirements

Candidates must possess at least 5 years of clinical practical experience and 2 or more years of case management experience. A Registered Nurse license that is unrestricted in their state of residence with multi-state/compact privileges is required, along with the ability to be licensed in all non-compact states. An Associate's degree is required, and a Bachelor's degree is preferred. Proficiency with standard corporate software applications, including MS Office Suite, and efficient computer skills are necessary. Willingness to obtain additional state RN licenses is also required.

Responsibilities

The Case Manager - Registered Nurse will act as a frontline advocate for members, assessing, planning, implementing, and coordinating case management activities to evaluate medical needs and facilitate wellness. Responsibilities include developing proactive care plans, using clinical tools for evaluation, applying clinical judgment to reduce risk factors, conducting holistic assessments, and collaborating with stakeholders to overcome barriers. The role involves utilizing case management processes in compliance with regulations and company policies, as well as employing motivational interviewing skills for member engagement.
