Requirements

  • Bachelor's degree in a Human Services field from an accredited college or university

Responsibilities

  • Assist program participants in identifying and accessing resources for independent healthy living
  • Collaborate with a multidisciplinary team to create and implement individualized Comprehensive Treatment Plans
  • Collaborate with community systems for referrals, crisis management, and advocacy
  • Provide direct services to children and families, including case management and daily living support
  • Evaluate services and make necessary improvements for increased outcomes
  • Maintain regular contact with program participants and serve as liaison to the team
  • Monitor participants' progress and update team members on treatment plan goals
  • Connect participants with community resources and family support
  • Conduct assessments to inform individualized treatment plans
  • Arrange transportation for community appointments as needed
  • Participate in organizational processes as directed by Leadership, such as quality assurance and safety activities
