Candidates must possess an unrestricted Registered Nurse (RN) license with active multistate (compact) status and no disciplinary actions. A minimum of 3 years of clinical acute care experience is required, including experience with case management, discharge planning, and patient education for adult acute care. Demonstrated clinical knowledge, expertise in managing chronic conditions, and the ability to develop and implement individualized care plans are essential. Strong clinical assessment, critical thinking, and autonomous decision-making skills are necessary. Intermediate to advanced proficiency in Microsoft Office applications (Word, Excel, Outlook) and the ability to navigate multiple computer systems with dual monitors are also required.
The Care Manager, Telephonic Nurse will assess and evaluate members' needs to promote optimal wellness through care coordination and by facilitating interaction with appropriate resources.
