Requirements

  • 1-2 years’ experience in case management, social services, or healthcare coordination
  • Strong knowledge of local healthcare, community, and social service resources (or a quick learner!)
  • Bilingual in Spanish or Vietnamese
  • Tech-savvy with solid computer skills (EHR experience is a plus)
  • Valid CA driver’s license & insurance
  • Physical Requirements: Comfortable sitting, standing, walking, typing, and occasionally lifting up to 25 lbs

Responsibilities

  • Be the first friendly voice for prospective ECM members, inviting them to join our program
  • Support clients and their families by scheduling, coordinating, and keeping everyone on the same page
  • Work with our Lead Care Managers (LCMs) to educate clients on all the ways we can support them
  • Help clients access important community resources and benefits (like SSI, CalFresh, or Community Supports through CalAIM)
  • Be part of a collaborative care team, sharing updates and brainstorming solutions to meet patient needs
  • Make periodic check-ins with clients to cheer them on and help them reach their health goals
  • Play a key role in helping patients transition smoothly from hospital or institutional settings back into their communities
