  • Bilingual in Spanish (Spanish speaking)
  • Ability to perform remote work (90% of duties from home)
  • Willingness to conduct 1-2 on-site community visits per month near your city

Responsibilities

  • Primarily work with and support a caseload of individuals with complex medical needs
  • Work with individuals to identify health needs
  • Contribute clinical expertise in a team-based model with lead care manager community health worker, behavioral health care manager, and nurse practitioner
  • Support patient-driven care plans to drive improved outcomes for individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs
  • Provide health education and complex care management to improve individuals' quality of life
  • Engage in 1-2 in-person on-site visits per month in the community alongside a fellow PairMate
