Requirements

  • Active Nurse Practitioner or Physician Assistant license in good standing or the ability to obtain licensure in multiple states within 90 days of hire
  • Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
  • Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings
  • Active BLS certification at time of hire
  • Shift times may vary to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone; flexibility required. Weekend shifts may be required in the future
  • Bilingual (Spanish/English)

Responsibilities

  • Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions
  • Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans to optimize patient health
  • Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) within the scope of virtual care
  • Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary
  • Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety
  • Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health
  • Identify patients needing referrals or home-based services, and facilitate appropriate coordination with specialists or community resources
  • Utilize and provide feedback on the electronic health platform, contributing to its continuous improvement to enhance patient and provider experience
  • Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care
  • Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management
