Requirements

Active Nurse Practitioner or Physician Assistant license in good standing or the ability to obtain licensure in multiple states within 90 days of hire
Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings
Active BLS certification at time of hire
Shift times may vary to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone; flexibility required. Weekend shifts may be required in the future

Responsibilities

Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions
Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans to optimize patient health
Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) within the scope of virtual care
Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary
Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety
Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health
Identify patients needing referrals or home-based services, and facilitate appropriate coordination with specialists or community resources
Utilize and provide feedback on the electronic health platform, contributing to its continuous improvement to enhance patient and provider experience
Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care
Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management
