Requirements

Bilingual Licensed Clinical Social Worker (LCSW)
Ability to work remotely with 1-2 on-site community visits per month near your city (90% remote)
Expertise in providing mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs
Ability to work in a team-based, interdisciplinary model with lead care managers, community health workers, registered nurses, and nurse practitioners
Highly motivated, creative, and empathetic mindset
Full-time availability

Responsibilities

Primarily work with and support enrolled individuals who struggle with severe mental illness or substance use disorders
Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
Contribute expertise to improve individuals' quality of life through evidence-based behavioral interventions
Participate in a whole-person, interdisciplinary care model
Conduct 1-2 on-site visits per month in the community alongside team members
