Responsibilities

Primarily work with and support enrolled individuals who struggle with severe mental illness or substance use disorders

Provide expertise in a team-based model alongside lead care manager, community health worker, registered nurse care manager, and nurse practitioner to improve individuals' quality of life
Conduct 1-2 on-site visits per month in the community with a team member
Contribute to whole-person care for individuals with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or high medical needs
