Must reside in or near zip codes 33125, 33126, 33165, 33130, 33133, or 33135 in Miami-Dade County
Willing and able to travel 50-75% of time for face-to-face visits with members in Miami-Dade County and surrounding areas in Florida
Bachelor's degree required, in Social Work or related field

Responsibilities

Telephonically and/or face-to-face assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness
Effectively manage a caseload that includes supportive and medically complex members
Develop a proactive course of action to address issues presented to enhance short- and long-term outcomes and opportunities to enhance a member's overall wellness through integration
Determine appropriate services and supports due to member's health needs, including but not limited to prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources, and supports
