Requirements

  • Bachelor’s degree in social work or psychology-related field (unlicensed) or certified coach with experience in mental health
  • Computer and live video tools literacy (e.g., Zoom, Google Meets, DoxyMe, Skype, etc.)
  • Organized and excited by a dynamic environment
  • Passionate about helping kids and teens achieve goals
  • Ability to work full-time, approximately 40 hours per week (Monday - Friday 11:30 AM - 8PM PST / 2:30 PM to 11 PM EST)
  • Candidates must successfully pass a drug screen and background check prior to employment
  • Full-time resident of the United States
  • Additional language skills are beneficial to this role

Responsibilities

  • Primary point of contact for members and their care team providing initial evaluations, screening, and triage to appropriate services
  • Track and monitor care needs
  • Ensure care is on track and care needs are being met
  • Build relationships with external providers with timely responses and involvement in care planning
  • Provide accurate and timely documentation in the EHR and other communication systems
  • Facilitate referrals for clinically indicated services outside the organization
  • Create accurate discharge summaries and plans to ensure members and families have the services and support needed for ongoing care
  • Schedule sessions with the internal care team to ensure continued progress toward care goals
  • Participate in training on our proprietary curriculum and collaborative care model
